Communicate diabetes regimen changes to primary team and nursing staff. Review, add, discontinue and/or modify diabetes orders in EMR. Document the effectiveness of current diabetes regimen in the medical record using subjective and objective data. Establish the diabetes follow-up plan to expedite discharge. Consult dietitians and diabetes educators as appropriate. Provide consultation to the diabetes educators regarding after discharge follow-up of blood sugar control. Participate in the development, utilization and evaluation of resources to meet patient and family educational needs. Assure clinical competency and expertise in diabetes management. Collaborate with medical, nursing, nutrition, and other health professionals to monitor and evaluate the quality of care of hospitalized patients with diabetes. CCRES Educational and Behavioral Health Services was created in 1999 as an independent IRS 501(c)(3) nonprofit organization through a partnership with the Chester County Intermediate Unit to meet the growing need for reliable, professional, educational and behavioral health staff for children in the county, and as a vehicle to give back to the community. CCRES has grown and now provides vital support for children, adolescents, families, and adults with a range of exceptionalities, including autism, behavioral health concerns, mental illness, as well as intellectual, developmental, and physical disabilities. CCRES also provides a variety of experienced school administrators to educational agencies throughout the region. CCRES has grown over the years to provide services in Berks, Bucks, Cumberland, Montgomery, Delaware, Lebanon, and Lancaster Counties.Reporting to the Board of Directors, the Executive Director will lead CCRES' mission to provide staffing for educational and behavioral health services to school districts, intermediate units, and other educational entities throughout the Commonwealth of Pennsylvania. The Executive Director will inspire and lead a team to achieve the highest levels of excellence so that each individual served is provided with a meaningful and rewarding education. The Executive Director builds and maintains relationships with partners including Superintendents/Executive Directors of school districts, intermediate units, and other educational entities in order to provide quality services to meet their needs.Qualified candidates will possess a minimum of 10 years of experience providing executive level leadership for educational, nonprofit, government, or business organizations; special education or behavioral health experience preferred. The successful candidate must have professional experience in human resource management, policy development, financial management, and contract negotiations. Transformational and servant leadership skills and organizational management experience with the ability to lead and build consensus within a complex, diverse organization are essential. Must possess strategic planning capabilities and the leadership necessary to ensure its successful implementation. Proven experience in fiscal management and preparing and securing Board approval for a complex budget is required. Master's degree in education, public administration, business administration, or related field required. The Executive Director is expected to be onsite Monday through Friday in Downingtown, PA, as well as travel and meet with current and prospective clients.
