Arrange appointments for medical visits, social security office, school enrollments, legal services, etc.Aid with filing for disability, food stamps, FMLA and other services.Perform outreach and connect with community organizations as needed based on clients' medical conditions. Assist clients with insurance applications, provide health care and medical system orientation. Coordinate Group-Only Programming.Assess the need for client direct assistance, distribute and keep record of it.
