The MGB Home Hospital (HH) is an innovative strategic program that aims to improve the care of patients while increasing hospital capacity and reducing total costs of care. The Home Hospital Advanced Practice Provider will support the work of Home Hospital by visiting (either in person or virtually) and managing patients in their home as a direct Home Hospital provider, supported by a multidisciplinary team that includes an attending physician (who oversees the direction of care for patients enrolled), a Home Hospital nurse, a paramedic (Mobile Integrated Health), an administrative coordinator, and other allied staff as needed (e.g. home health aide, physical therapist). NURSE PRACTITIONER, M.S./ 40 HOURS/ DAY SHIFT/ BWH Primary Care- InBasketAbout this role:The Nurse Practitioner will work in a remote / virtual capacity as part of the team covering Primary Care work curated for them within their in-basket folders. Specifics of this work will be defined over the roll-out of this initiative and will change over time. Telephone encounters/messages will range from routine to emergent and will require patient assessment, diagnosis, outcome identification, planning, implementation, and evaluation. The NP will work to resolve Patient Gateway messages in real time by either providing clinical advice or offering a virtual visit. The NP will place orders for testing, vaccines, referrals, or refills following refill protocols, not including controlled substances. They will be responsible to follow up on orders placed. Communication within pools with PCPs, nurses, medical assistants, practice administrative support, and practice leadership will be required. The NP will be required to meet virtual visit/phone call/message metrics as set by Primary Care.GENERAL SUMMARY/ OVERVIEW STATEMENT:The Nurse Practitioner is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care. Schedule: Per Diem- Weekdays Only- 2 days per week initially and then 1 shift per monthThe Palliative Care Advanced Practice Provider (APP) is a core member of the BWFH Department of Medicine and key constituent of the palliative care division. The APP is primarily responsible for serving as a palliative care consultant for inpatient hospital services- mainly medicine, critical care, and emergency department. Candidates for the position should have a strong interest in palliative care, and ideally, experience and training in palliative care. The palliative care APP will report to the BWHF division chief or palliative care to develop all necessary clinical skills in a highly collaborative and supportive environment. PRINCIPAL DUTIES AND RESPONSIBILITIES: The APP works closely with the palliative care physicians, inpatient physicians and advanced practice professionals, residents and nursing staff to develop and coordinate a plan of care designed to meet the physical, psychological and social needs of patients and their families.
