(approx. 18 days per quarter)   Pay Range: Salary: $145,000 - $155,000 with up to 30% annual bonus potential Essential Duties and Responsibilities: Provide healthcare chronic disease management services Develop and/or follow protocol based healthcare plans for geographically separated patients Work collaboratively with all healthcare continuum providers Assist geographically separated patients with little to no access to healthcare Travel and meet new people while having new experiences Occupational health exams to include pre-and post-employment and annual physicals Generalist approach to multi-system disability evaluations includes evaluations of conditions specific to the military operational environment like Gulf War and Prisoner of War. Occupational health/ disability examinations are in-person evaluations, but some may be completed virtually or by medical record review only. Current ACLS certification Must have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions.  Must maintain a valid AZ driver license during employment.  Must be insurable by our liability auto policy. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment. Must pass a substance abuse testing upon employment and submit to a random testing during employment. Must have an updated Immunization Record prior to start of employment. Must have a current Tuberculosis skin test report upon employment and provide an update annually.    Must have a CPR Card prior to start of employment and maintain a valid CPR card during employment.
