Discharge planning will include assuring appropriate lower level of care is established or referrals will be made. There are three units in the UCC serving youth ages 4-18 (up to 21 if DCF committed). This is a fast-paced environment that operate 24/7, 365 days of the year. The Registered Nurse will work amongst a multidisciplinary team in the assessment, treatment, and discharge of the youth cared for in this Urgent Crisis Center.
