Each of about 6 CCPs has a panel of 200-250 patients each. These patients have a high degree of acuity and require specialized care from a multidisciplinary team. The team includes nurses, a nurse practitioner, social workers, program coordinators, and a community health worker, who meet daily to provide the best possible care for their patients.Job SummaryThe CCP Program Nurse will assist physicians in the CCP clinic in patient care activities. They will be responsible for managing clinic flow, including obtaining vitals, rooming patients, and appropriately allocating exam rooms to physicians. They will facilitate care coordination between multiple internal and external resources and also assist in assuring seamless transitions of care between inpatient and outpatient settings. The Nurse Case Manager (NCM) is the single point of contact for patients and their caregivers. They are aligned regionally and are responsible for educating patients, caregivers and families affected by generalized Myasthenia Gravis (gMG) about the disease and argenx's products and support services.
