Our staff use a culturally-affirming, person-centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. The model uses a multidisciplinary team approach alongside evidence-based interventions and recovery-oriented models to enable participants to reach their goals. The team includes the Team Leader, Primary Clinicians, Outreach and Recruitment Coordinator, Supported Education and Employment Specialist, Peer Specialist, Psychiatrist, and Registered Nurse. Services are either provided at the clinic site, online, in the home, or other off-site locations in the community. 24-hour on-call services are included. Intensive education will be offered to all members of the OnTrackNY Team regarding the evidence-based model and their roles for members to adhere to fidelity standards and successfully support program participants. The Primary Clinician/Outreach and Recruitment Coordinator is a dual role. It includes the role of clinician as well as the role of outreach and recruitment coordinator for the program. The ideal candidate partners with participants, families the interdisciplinary clinical team, and any systems involved in the participant's lives (educational, medical, social services etc.) to support reaching goals in a comprehensive way, while providing culturally affirming and curious services. The clinician must have a firm understanding of how trauma and race, class, and privilege impact families of color and must be willing to address these issues as part of the therapy process. The PC/ORC role also includes the screening and assessment of potential participants, rapid intake and enrollment, and partnering with the community to provide information about our services.Some responsibilities include, but are not limited to:Act as primary resource and offer support to participants and families through the OnTrackNY evidence based model and differentiate care through phases of treatment: initial engagement, ongoing treatment, and Graduation (discharge) from OnTrack Using active outreach and engagement to build relationships and maintain retention with participants and families Use OnTrack evidence based practices, such as Core Sessions and Recovery Coaching, as well as therapeutic approaches based on Strengths Based Case Management, Motivational Interviewing, and Cognitive Behavioral Therapy Assess for safety and risk using evidence based models and support in safety planning and relapse prevention planning Support participants holistically.
