Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resiliency of our communities.Facilitated Intake Team Social Worker provides comprehensive intake assessments and treatment planning for clients entering the Certified Community Behavioral Health Clinic (CCBHC) with the goal of assessing their needs and linking them to the services at the site that best fit their needs and/or linking them to programs out in the community. The social worker is responsible for ensuring that the client is connected to the services after referrals are made and assists the client with navigating the various systems of care they access.Some responsibilities include, but are not limited to:Provide information and clarification about services and identify what service(s) are being sought - outpatient clinic treatment, Personalized Recovery Oriented Services (PROS), care management and/or substance use disorders treatment in addition to other identified areasObtain demographic and clinical information regarding the client's relevant history and presenting concerns for the respective serviceProvide a comprehensive assessment of client's needs including mental health and substance use disorders treatment, financial, housing, legal and medical needsConduct crisis evaluationsPrepare and maintain intake logMake appropriate referrals for various levels of care both within the CCBHC and in the community as neededTrack and provide follow-up on current and pending applications and provide follow-up with clients who do not keep intake appointmentsAssess for risk, create safety plans and prioritize cases deemed high risk for treatment. ABOUT THE JOB:The Mental Health Consultant will function in an ACS borough office as the lead clinical consultant on mental health issues impacting families known to child welfare.KEY ESSENTIAL FUNCTIONS:Provide training to ACS staff in mental health assessment, diagnosis and service planning.In conjunction with the Team Coordinator and other consultant staff help develop a clinical team that provides coordinated consultation in cases where there is co-morbidity.Complete required documentation of clinical consults provided within the requisite time frames.
