Develops implements and evaluates a program of specialized health care for the Hematology Oncology and Infusion population in the MVH Infusion/MGH Hematology Oncology practice at Martha's Vineyard Hospital (MVH). Provides access for patients into the program and coordinates patient care throughout the care continuum. Promotes a collaborative relationship and effort between the medical staff, professional nurses and administrative staff, to ensure continuity of patient care and efficient use of resources. This is a Hybrid Position!Social Workers at MVH provide strength based, culturally responsive, comprehensive psychosocial assessments, crisis intervention and safety planning, short term clinical support, and information and referral, to patients and their families seen in our Emergency Department, Outpatient Primary Care, Specialty Clinics, and during inpatient hospitalizations. All services all located on the grounds of MVH. The RN Care Manager is an integral member of the health care team and is embedded in the primary care practice where patients are seen. The RN Care Manager collaborates with the team in the delivery of patient care services across the continuum of care. Patient's health risks and barriers to care are identified, individualized care plans are established to help coordinate care between providers and services, and linkages made between patients and external community resources.
