STAFF SPEECH LANGUAGE PATHOLOGIST / FULL TIME/ 40 HOURS / DAYS / BOSTON, MAThe Speech-Language Pathologist, with an unrestricted Massachusetts license, is responsible for providing evaluation and treatment services for patients referred with speech, language and/or swallowing disorders. Services are provided on an inpatient and outpatient basis. Rehabilitation Services at Brigham and Women's Hospital provides physical therapy, occupational therapy, speech and swallow services, and orthopedic technology services to patients on a consult basis to those on our inpatient floors, ICUs, and the Emergency Department. Staff speech language pathologists will be joining a team with a diverse background in clinical care and expertise. The primary responsibility of the staff physical therapist is to provide patient care in accordance to State licensing regulations, departmental standards, and policies, and accepted professional standards of care. In addition to regular collaboration with other rehab disciplines as well as the multidisciplinary team, therapists will have many opportunities for clinical growth through regular internal continuing education programming offered, financial support for continuing education courses and conferences. Practice Locations: Brigham and Women's Hospital, Dana Farber Inpatient Hospital within Brigham and Women's Hospital; potential for occasional ambulatory care. Principal Duties and Responsibilities:Provides skilled speech-language and swallowing services as assigned to patients at Brigham and Women's Hospital and at Dana Farber Cancer Institute in accordance with State Licensure Statute and Regulations, Professional Standards of Practice and Code of Ethics, applicable federal, state and regulatory agencies' standards and regulations, hospital and departmental policies and procedures. Specifically:Examines and evaluates patient's communication and swallowing function using Clinical Evaluations, Videofluoroscopic Oropharyngeal Swallowing Evaluations and Fiberoptic Endoscopic Evaluation of Swallow (FEES). Identifies speech, language and/or swallowing disorders and associated dysfunction(s). In collaboration with the patient and/or caregiver, develops a treatment plan with treatment goals directed towards maximizing function.Demonstrate competency in all clinical knowledge and skills specific to the patient case type(s) in assigned area of care.Consults with other members of the health care team to insure coordination and effectiveness of therapies provided.Assists in implementation of triaging, prioritizing, monitoring admissions, referrals, and discharges to and from the service as assigned. Manages patient caseload assigned.Demonstrates effective written and oral communication skills. GENERAL SUMMARY/ OVERVIEW STATEMENT: Under the supervision and direction of the designated supervising physician, and in accordance with the rules and regulations of Massachusetts governing Nurse Practitioner (NP) practice, the NP supports the inpatient Neurosurgical Service. The neurosurgical NP will work directly with the clinical faculty and residents to provide quality patient care and medical services as it relates to all phases of neurological surgery for comprehensive management of brain, spinal cord and peripheral nerve diseases and disorders. Reports to the collaborating neurosurgeon.PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Order Writing1.1 Orders for all situations covered by protocols are written and signed by the NP including his/her title.1.2 Orders for situations not covered by protocols are first discussed with an appropriate physician involved in the patient's care and written by the NP and including the physician's name.2. Preoperative Patient Care (Inpatient)2.1 Elicit a detailed and accurate history, perform an appropriate physical examination, record and present pertinent data in a manner meaningful to the physician.2.2 Perform and prescribe routine laboratory and related studies, medications, and consultations standard for the Neurosurgical Service. Admission and preoperative orders will be entered into Order Entry and signed by the NP. The admission orders will include routine laboratory tests, consultations and continuing medications, which the patient had been taking prior to admission.3. Postoperative Patient Care3.1 The NP will alternate with a junior surgical resident and additional Nurse Practitioners and/or Physician Assistants in the management of neurosurgical patients. Under advisement of a resident or an attending physician, they will prescribe/order prn medications, treatments, routine labs, and consults.3.2 Perform suture removals and wound checks on as needed basis.3.3 They are to be qualified to:§ insert ventriculostomies or ICP bolts under the direct supervision of a physician§ discontinue ventriculostomies or ICP bolts3.4 Neurosurgical CareA. To assist in implementation of the treatment plan as formulated by the attending neurosurgeon. It is expected that the NP will contribute to the assessment of the patient, respond appropriately to the clinical situation with good clinical judgment, and, with the attending and resident staff, develop and implement the clinical care plan, within the scope of the practice.B. Participate in daily rounding on the inpatient neurosurgical units through multidisciplinary collaboration.C. Assist in facilitating patient movement through the system, particularly as it relates to the admission and discharge process.D. Independent performance of evaluative and treatment procedures essential to provide an appropriate response to life threatening emergency situations per Neurosurgical Service guidelines. 3.5 EducationA. To assist in the education of the patient regarding his/her illness, surgical procedure, and careB.
