Coordinate holistic healthcare for children in MATCH program by partnering with the foster parents, child welfare caseworkers, biological parents, managed care organizations, and healthcare providers. Ensure the health care needs of children in foster care are being met in a timely manner. Identify children at risk for health issues/exacerbation of illness and monitor closely. Attends weekly team meetings to review case assignments and transfers. Provide resources, recommendations, and education to caregivers. Complete home visits as necessary to assess children’s health needs and provide education to caregivers. Assist foster care families with navigating the Health Choice system in establishing a medical home for children in the Baltimore City Department of Social Services (BCDSS) foster care system. Prioritize care and contacts based on urgency- including ability to multi-task with accuracy. Document case management assessments, activities, treatment plans and resolutions in BCDSS and the MATCH Program database. Participate in Family Involvement and Team Decision Meetings as required. Participate in staff, community, professional and inter-agency meetings and conferences. Other job duties as assigned. A Bachelor’s degree in nursing from an accredited college/university Current licensure as a Registered Nurse in the State of Maryland.  Relevant clinical experience providing services to families and children, preferably in a public health/community setting.
