Provide Crisis services in the community as needed. The MH Therapist- Mobile- Crisis will provide clinical services to adults on-site at the clinic and offer crisis services in-home and in the community.DUTIES AND RESPONSIBILITIES:Work with clients and their families on treatment adherence issues with medications and maintaining appointmentsConducts assessment/reassessments of client service needs and priorities every 90 days.Develops (in conjunction with client) appropriate treatment plans based on assessment.Case conferencing with mental health providers to determine goals/objectives, activities and who will perform needed tasks related to mental health issues for client and familyCollaborative and concurrent documentation requiredProvide individual/group therapyAdminister appropriate behavioral interventions as needed (CBT, Seeking Safety, motivational interviewing etc.)Maintains accurate and up-dated records in accordance with the programs policies and procedures.Crisis intervention (aggressive behavior; suicide threats, ideation; domestic violence) work with clients who are experiencing domestic violence (DV) or have a history of DV, and/or who have co-occurring mental illness, life trauma or chronic conditions and/or substance abuse issues)Other related dutiesKNOWLEDGE, SKILLS AND ABILITIES REQUIRED:Strong individual and group counseling skills.
