How you can make a bigger difference: The Inwood ACT program provides an integrated set of evidenced based treatment, rehabilitation, case-management and support services to individuals residing in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care. As a member of the ACT team, the RN is responsible for providing a range of treatment to clients with a SPMI, rehabilitation and support services within the scope of the nursing practice for the clients receiving services from ACT. S/he is responsible for conducting initial nursing assessments and ongoing health monitoring assessments ; assessing physical health needs; making appropriate referrals to community physicians as needed, and providing management and administration of medication in conjunction with the prescriptions/orders of psychiatrist and PNP. The majority of this role is conducted in the community and is not site specific. The RN will be supervised by the Program Director. Developing a treatment plan based on clients' needs. Ensure that any necessary follow-up will occur and any necessary or prescribed care is administered. Completing nursing assessments and health monitoring assessments according to OMH standards of care within appropriate timeframes. Will follow/implement all direction from Psychiatrist or PNP regarding medical treatments and medication orders. Will administer prescribed and/or routine medical treatments, support clients and staff with self-administration of medication procedures and evaluate for potential side effects or adverse reactions. Ensuring that all internal policies regarding such administration are adhered to. Will coordinate and deliver medications to the client's residence or shelter, be responsible for medication counts, follow-up the pharmacies (i.e. refills, insurance authorizations etc) and administering medication to clients. Making referrals, schedule appointments and accompany the client when required. Making visits to hospitalized clients to coordinate care. Provide advocacy for hospitalized clients.
